In order to avoid other air traffic, co-pilot Andrew Eaves asked for permission at approximately 8:46 p.m. for the helicopter’s pilots to use their own visuals rather than air-traffic control.
Since it’s standard procedure to help expedite things, this was permitted; nonetheless, human mistake is a concern.
Eaves, who was in charge of her training trip, also gave Rebecca Lobach a directive that she failed to follow.
Additionally, authorities discovered that the pilots had “stepped on” a few of the ATC’s directives, which means they accidentally turned off Eaves when they pressed the button to communicate via radio.
They are thought to have missed crucial information as a result of this.
